Making a form work?

I have a form on my website and I want to gather mailing information. Not email information. I created a form but I can't seem to have it send the information to the email. When I test the form it acts like I have gathered the information but nothing shows up in the email address. I don't know where it's going.??

Can someone help me to figure out what is wrong please?

Looks like you have an error in your form action.  It should point to a php file that has the script needed to take your form data and do something with it.  If you want to email the form results, this tutorial will help you understand what needs to happen: http://myphpform.com/final-form.php. That being said, I am struggling with forms myself, but found that source to be helpful in understanding what is going on behind the form. Good luck.  Hope you get some good feedback.

You need to evaluate the code  of your submit button function. Debug the coding and check whether the correct insert operation get executed on submit button or not. Once you solve your problem, check your table in the server where you store all email address manually and confirm that insertion takes place in the database.
