DbQwikReport is (or was) a commercial (paid) product. You will probably get your best support from its maker. If you need to create a MySQL database, your control panel (cPanel or LPCP) should have a section to create a database, name it, create a user and password, associate that user with that database, and grant desired privileges to that user. Then you have to tell the product or application (DbQwikReport) the database name, location ('localhost' or a name or IP address, depending on what server you're on), user name, and password. The database name and user name are usually going to be prefixed by your account name and an underscore. E.g., if you create database 'glorp' and your account is 'yabbadabba', the database name you give to the application is 'yabbadabba_glorp'. At that point, your application should be able to create and fill the tables in the database.
